Prescription Refill Policy
To ensure patient safety and proper medication monitoring, refill requests are handled according to strict clinical guidelines:
- Advance Notice: Please submit maintenance medication refill requests at least 48 to 72 business hours before running out of medication.
- Preferred Method: Request refills directly through your secure patient portal or have your pharmacy send an electronic refill authorization request directly to Cedar Grove Health.
- Routine Monitoring: For medications treating blood pressure, diabetes, thyroid, or cholesterol, an in-person or telehealth follow-up visit is typically required every 6 to 12 months to ensure optimal dosing and check routine safety labs.
- Controlled Substances: Regulations require scheduled follow-up consultations before authorizations for controlled medications can be issued. Refills cannot be authorized after-hours or on weekends.
Medical Records Requests
Your health records are protected by federal HIPAA regulations. To transfer medical files from Cedar Grove to another provider or to request records from a previous clinic:
Step 1: Download and sign our Medical Records Release Form (PDF).
Step 2: Deliver the signed form via fax to (555) 382-4101, upload it through your patient portal, or bring it to any clinic reception desk.
Processing Time: Standard medical records transfers are completed within 5 to 7 business days. Urgent requests for active specialist appointments are expedited.
How You Receive Lab Results
Routine blood work (CBC, metabolic panels, lipids, HbA1c) is processed within 24 to 48 hours. Once your physician reviews the values and attaches notes, you will receive an automatic SMS or email notification that results are available on your portal.
