Frequently Asked Questions
Quick answers regarding appointment scheduling, insurance verification, prescription renewals, medical records, and clinic visits.
Scheduling is easy! You can book online 24/7 through our interactive booking scheduler on this website, or call our central intake desk at (555) 382-4100 Monday through Friday from 8:00 AM to 6:00 PM. We will collect basic demographic information, verify your insurance, and send you digital intake forms.
An annual physical focuses on total-body preventive health, reviewing risk factors, vital benchmarks, updating vaccines, and ordering age-appropriate screenings before symptoms arise. A sick visit is focused on diagnosing and treating a specific acute illness or flare-up (e.g. fever, cough, earache, or sudden back pain).
We participate with major commercial payers including Blue Cross Blue Shield, Aetna, Cigna, UnitedHealthcare, Medicare Part B, Humana, and select state Medicaid managed care networks. You can search our live accepted insurance directory on our Insurance page or call our billing team at (555) 382-4140.
We offer transparent, predictable flat self-pay fees under our Good Faith Estimate policy. An acute same-day sick visit is $95, an established patient follow-up is $115, and a new patient comprehensive intake is $175. We also offer 0% interest monthly payment plans.
The fastest way to request a routine maintenance refill is through your secure Cedar Grove Patient Portal or by contacting your pharmacy directly to submit an electronic renewal request. Please submit requests 48 to 72 business hours in advance.
Yes! All three Cedar Grove Health centers have on-site specimen collection stations during morning clinic hours. Routine results are reviewed by your physician and published to your portal within 24 to 48 hours.
Telehealth visits allow you to consult face-to-face with your physician via smartphone, tablet, or laptop. Fifteen minutes before your visit, you receive a secure link via text and email. Simply click the link in your web browser—no application download is needed.
Download our Medical Records Release Form from the Patient Resources page, fill out your previous clinic's contact details, sign the HIPAA consent, and fax it to (555) 382-4101 or bring it to our front desk. We will coordinate the record transfer on your behalf.
For urgent clinical concerns outside regular office hours, call our main clinic number at (555) 382-4100 to connect with our on-call registered nurse triage hotline. If you believe you are experiencing a life-threatening medical emergency, call 911 immediately.
We understand schedules change! We ask for at least 24 hours advance notice if you need to cancel or reschedule your visit so another patient can be offered that time slot. You can reschedule easily through your portal or by calling our desk.
